Release of Liability:
I hereby release, discharge, and agree to hold harmless Kate Mulleneaux and Moments, LLC, and any persons acting under their permission or authority, from any liability, claims, or demands arising out of or relating to any blameless accident, illness, or injury sustained by the minor (or parent/guardian present) during the photo session or any subsequent use of the photographs.
Acknowledgment:
I understand that I will not receive any monetary compensation for the use of the photographs and that this agreement is binding and final. I have read this release before signing below and fully understand the contents, meaning, and impact of this release.
I understand that Moments, LLC will release all photos without copyright to me in a digital Google Photos album within one week from today's date.
I understand that post-production processing is not included in the product price and will incur additional charges based on the required services. Pricing details are available on the website for reference.
I understand that for on-site locations beyond a 10-mile radius from the 80004 zip code, an additional fee of one dollar ($1.00) per mile will be added to the service cost.
I understand that any cancellation or postponement of services will result in the forfeiture of any deposit paid. Additionally, repeated no-call, no-show incidents will lead to the termination of future services.
I understand that once the photo album is released to the client, no refunds will be issued.
