I, the undersigned, hereby grant Moments, LLC, Kate Mulleneaux, permission to use photographs and/or video recordings of me and minors under my supervision taken during the designated time slot as agreed upon by client and photographer, for the following uses:

Release of Liability:

I acknowledge and agree that participation in the photography session is voluntary and at my own risk. I hereby release and discharge the Photographer, its agents, employees, and representatives from any and all claims, demands, and causes of action that I and/or any individual accompanying me on my photography session, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. This includes but is not limited to any claims for personal injury, property damage, or any other type of loss or damage arising out of or in connection with my participation in the photography session.

Consent to Use:

I understand and agree that the photographs and/or video recordings may be edited, copied, exhibited, published, or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising from or related to the use of the images or recordings.

Revocation:

I understand that I may revoke this consent at any time by notifying the Photographer in writing. However, I understand that revocation will not affect any images or recordings that have already been used or published prior to the Photographer's receipt of my revocation notice.

Acknowledgment:

I have read this release of liability and consent to use form and fully understand its contents. I am at least 18 years old, as indicated above, or I am the parent/legal guardian and have full legal capacity to execute this document. I also understand that by signing this form, I am signing it on behalf of all people / individuals / pets / other specifics listed above.

Deposit and other specifics:

I understand that there is a required non-refundable $50.00 deposit that goes toward the final cost of service. The rest of the service rate is due at time of service. Photography sessions do not include printed photographs or edit/post -production processing. As a courtesy, please try to cancel no later than 24 hours prior to a shoot. No-call, No-show appointments will not be able to reschedule.
